It's day three in the ward. Tito did the first night, Mama did the second, and your brother has work at 7 a.m. Lolo keeps trying to pull at his IV line when nobody's looking. In the group chat, someone finally types it: "Kumuha na lang kaya tayo ng bantay?" Should we just get a watcher?

Hiring a watcher isn't giving up on family care. It's how families keep going through a long admission without everyone burning out. Here's what to expect, what to ask, and how to set the shift up so it goes well.

What a hospital watcher does, and doesn't do

Hospital companion rules: check first, every time

There is no single Metro Manila rule for watchers. Each hospital sets its own, and they change. Some public examples:

  • Olivarez General Hospital (Parañaque) says only one adult companion is allowed per patient, with two possibly allowed for small pediatric, elderly and disabled patients. It issues a watcher ID on admission, which must be worn inside the hospital, and charges a replacement fee if it's lost.
  • Makati Medical Center announced in 2020 that it issues one companion ID pass, through the Admitting Office, for the companion staying beyond visiting hours. That announcement is from 2020, so confirm the current rule.
  • St. Luke's Medical Center lists visitor limits on its site, including only two visitors per patient allowed in after 10:00 p.m., and one visitor per patient inside some units such as the PICU.

So before the first shift, ask the nurses' station or admitting:

  • How many watchers are allowed, and can a paid watcher hold the companion ID?
  • How is the ID handed over between family and watcher at shift change?
  • Are there rules for ICU, isolation or special units?
  • Can the watcher bring food in, and where can they eat or rest?
  • Who should the watcher speak to if something changes?

When comparing quotes, use the same shift length, and ask whether meals and transport are included. A late-night shift change can mean a costly ride home.

Before the first shift: a handover checklist

A watcher can only help with what they know. Give them a one-page handover, or pin it in a chat thread.

About the patient

  • Name, age, what they're admitted for (in plain words)
  • Allergies and any "do not" instructions from the nurses
  • Diet: NPO (nothing by mouth), soft, low-salt, or as served
  • Mobility: can stand alone, needs one person, or bed-bound
  • Fall risk or confusion at night, and what calms them
  • Language or dialect they're most comfortable in

About the family

  • One main contact person, plus a backup number
  • What to message about (meals eaten, sleep, doctor's rounds) and what to call about immediately
  • Who can make decisions if the doctor asks
  • Whether cash is needed for small purchases, and how it's tracked (receipts, photos)

About the room

  • Where supplies are: diapers, wipes, tissues, water, extra clothes
  • Where valuables are, or better, take them home
  • Charging spot for the watcher's phone

What to pack for the watcher's shift

Diapers and wipes in the patient's size, a thermos, a jacket (wards get cold), slippers, a small notebook and pen for logging, and a power bank. Ask the hospital before bringing in food.

During the shift: what good looks like

A good watcher sends short, regular updates. For example: "Kumain si Lolo ng kalahati. Nag-round si Dr. ___ ng 9, sabi po ituloy lang. Nakatulog na." (Lolo ate half. The doctor did rounds at 9 and said to continue. He's asleep now.)

Agree on these upfront:

  • Update rhythm. Every few hours, plus after doctor's rounds.
  • Escalation. The watcher calls the nurse first for anything clinical, then tells the family right away.
  • Breaks. On a 12-hour shift, the watcher needs to eat and use the CR. Agree who covers, or ask the nurse to look in.
  • Shift change. Overlap 10–15 minutes so the incoming person hears what happened.

Choosing the right watcher

Look for someone who has done hospital shifts before, is comfortable with lifting or turning (if needed), and communicates clearly. For a longer stay, try to book the same one or two people so Lolo isn't meeting strangers every night.
